The custom started during the Middle Ages when the plague was in London. No one was certain of how it was spreading so the Mayor made the requirement in June of 1665 that all graves be at least six feet deep in order to limit the spread of the plague.

It was later determined that the plague was caused by rats and transmitted by the fleas that bit these rodents and then people.

The great fire that destroyed London killed most of the rats and so the disease was contained.

There are still rats around but why not the disease?

Interesting IG. To answer your last question I am wondering if the fires killed the germs from the plagues.

Why are graves facing East? (In USA they are assuming other countries as well)

Fire killed the rats with the fleas, therefore the source of the disease was eliminated.

Many answers can be found, one is that Christians believe with the Second Coming Jesus will come from the east.

Some believe the sunrise denotes a new beginning.
In cemeteries I have visited it is often determined by the layout of the graveyard and some graves are in all directions, although many are facing east.
